Membership hold requests are for 30 day increments and can't exceed 90 days per calendar year.
Please fill out this form if you plan on traveling or are injured and would like to put your membership on hold instead of cancelling.
This will serve as your 10 business day written Hold notice. Note that if you have a scheduled renewal payment within this 10 business day period, the payment will be processed as scheduled. All payments are non-refundable.

Reason for your requesting a Hold on your Membership:

I acknowledge that I am providing CrossFit Osiris a 10 business day written notice to place a hold to my membership. I understand that I am responsible for any billing should I fall within the next months billing cycle.(Required)

I acknowledge & I understand that I won't be able to enjoy the benefits of the facilities and Events while my membership is on hold.(Required)

I acknowledge & I understand there is a fee of $45 for each month I am on hold.(Required)
